The information booklet I sent earlier (“Trip Info & Checklist”) also contains several forms at the end. I will need the medical and disclaimer forms prior to departure.

The form I need most urgently is the Traveller Information & Copy of Passport form. Could you please complete this and send it back to me as soon as possible, together with a scan of your passport (with at least six months’ validity from your date of arrival in Indonesia)?

I now need to book the domestic flights between Jakarta and Papua for all guests travelling on this year’s expeditions, and I cannot finalise these bookings without this information.
